Manulife: new dental policy number

To all – please note effective July 1, 2016, we have new Dental policy number under our Manulife employee Group benefits plan. The Healthcare portion of the extended benefits remain under the current policy number.

Note only the policy numbers has changed, all the plan benefits remain the same.

The two Manulife plan numbers are as follows:

• Dental Care policy number 104402
• Health Care benefits 0030273

 

Please make sure you update this new plan information with your dentist.
